FAMILIES

DOCUMENTS NEEDED AT REGISTRATION AND PICK UP:

 -Driver’s License or State ID 
 -Proof of Residency with address provided in registration
 -Birth Certificate for ALL CHILDREN
 -Social Security card for ALL CHILDREN
 -Proof of Dependency (Only 1 of these items is required)
          -You are the Mother or Father listed on the Birth Certificate with ID.
          -Court Document listing you as the provider for the child matching the Social Security Card
          -Notarized Document from one of the parents listed on the Birth Certificate, authorizing you to sign
           and receive toys from this program on their behalf.

 *****Bringing all of these documents will be greatly appreciated and provide a smooth transition in getting toys delivered. Thanks in advance!!!
